Telehealth Notice & Informed Consent.

This Telehealth Notice explains how telehealth works if you choose to use telehealth-related features on the Menovia Platform. Please read it carefully before agreeing to use telehealth services.

1. Telehealth is Optional

Telehealth services (where available through the Menovia Platform) are entirely optional. You can continue to use all non-telehealth features of Menovia — symptom tracking, Zeina, community, education — without ever using telehealth.

You are never required to book a telehealth visit or share medical information with a Telehealth Provider to use Menovia.

2. Who Provides Telehealth Services

  • Telehealth services, when offered, are provided by independent licensed healthcare professionals and/or third-party telehealth organizations ("Telehealth Providers").
  • Menovia is not a healthcare provider and does not practice medicine. Menovia provides the Platform to facilitate access, scheduling, and coordination with Telehealth Providers.
  • Menovia is not responsible for the clinical decisions, diagnoses, treatments, or prescriptions made by Telehealth Providers.
  • Each Telehealth Provider has its own privacy practices, terms of service, and clinical policies. You will be required to review and agree to those separately when booking a visit.

3. No Emergency Services

DO NOT USE TELEHEALTH OR ZEINA™ FOR EMERGENCIES.

Telehealth Providers are not equipped to handle emergencies. If you experience any of the following, call 911 or go to an emergency room immediately:

  • Chest pain, difficulty breathing, or signs of a heart attack or stroke
  • Severe bleeding, severe pain, or loss of consciousness
  • Suicidal thoughts or thoughts of harming yourself or others
  • Severe allergic reactions
  • Any condition you believe requires immediate medical attention

If you are in crisis or considering self-harm, you can also call or text 988 (Suicide and Crisis Lifeline) in the United States.

4. Nature of Telehealth Services

Telehealth may include any combination of the following, depending on the Telehealth Provider:

  • Video or audio consultations
  • Secure messaging between visits
  • Remote questionnaires, intake forms, or symptom assessments
  • Care coordination and follow-up scheduling
  • Prescription consultations (where legally permitted)
  • Lab orders or referrals to in-person care

Your Telehealth Provider will determine what services are clinically appropriate for you and whether an in-person visit is needed at any point.

5. Risks and Limitations of Telehealth

  • Technology failures: Connectivity issues, audio/video disruptions, or device problems may interfere with a visit or delay care.
  • Limitations of remote evaluation: Without a physical exam, some conditions cannot be fully evaluated. Your Telehealth Provider may need to refer you for in-person care.
  • Privacy risks: Electronic communications, even when encrypted, carry some inherent risk of interception or unauthorized access. Use a private, secure network when attending visits.
  • Delays in care: Telehealth response times may be longer than in-person urgent care or emergency room visits.
  • Geographic limitations: Telehealth Providers may only treat patients located in states or jurisdictions where they are licensed.

6. Privacy and Medical Records

  • Telehealth Providers maintain their own medical records and privacy notices. Your medical records from a telehealth visit are held by the Telehealth Provider, not by Menovia.
  • Menovia may receive limited information necessary to coordinate services — for example, appointment status, basic intake responses you choose to share, and whether a visit was completed.
  • If you choose to attach a Menovia symptom report to your telehealth visit, you authorize Menovia to share that report with the Telehealth Provider.
  • You are responsible for using secure networks and protecting access to your device during telehealth visits.
  • Telehealth Providers' privacy practices are described in their own notices and may be governed by HIPAA, state law, or other regulations applicable to licensed healthcare providers.

7. Prescriptions

Where Telehealth Providers can prescribe medications:

  • Prescriptions will only be provided when clinically appropriate and legally permitted in your state.
  • Not all conditions are appropriate for telehealth prescribing. Your provider may decline to prescribe and refer you to in-person care.
  • Controlled substances may be restricted or prohibited via telehealth depending on federal and state law.
  • Hormone Replacement Therapy (HRT) prescriptions, where offered, will follow each provider's clinical protocols and applicable state regulations.
  • You are responsible for filling prescriptions at a licensed pharmacy and following the prescribing provider's instructions.

8. Location and Cross-State Practice

Telehealth Providers may only provide care in states and jurisdictions where they are licensed and where telehealth practice is permitted by law.

  • You may be asked to confirm your current physical location at the time of each telehealth service.
  • If you travel to a state where your Telehealth Provider is not licensed, telehealth services may be paused or unavailable until you return to a permitted state.
  • If you relocate permanently, you may need to find a Telehealth Provider licensed in your new state.

9. Informed Consent

By selecting "I Agree" within the Platform — or by using telehealth features — you acknowledge and consent that:

  • You understand the nature, benefits, and limitations of telehealth services as described in this Notice.
  • You understand that telehealth is not a substitute for emergency care or in-person evaluation when needed.
  • You understand that Telehealth Providers, not Menovia, are responsible for all clinical decisions about your care.
  • You can withdraw consent at any time by discontinuing your use of telehealth features. Withdrawing consent does not affect care you have already received.
  • You will provide accurate, complete, and current information to Telehealth Providers.
  • You have had the opportunity to ask questions about telehealth and have those questions answered to your satisfaction.
